lives
2. Original
Birth Certificate
- not hospital papers with footprints
3.
completed
Tournament Affidavit
4.
All II(d) and IV(h) forms
5. A check made out to District 27 -
$50 per team
6. Medical Release Forms (signed by parents)
7. Waivers approved to play All Stars
8. Your pink copy of the receipt that
shows you paid your fees to LL

all Tournament Rules.
